Mental contamination: feeling dirty that no shower reaches

A memory, an image, a person, a moral trigger, and suddenly you feel tainted. No contact, nothing on the skin, and the feeling is as real as any germ. It is OCD, and it is treatable.

Nothing touched you. There was no door handle, no toilet, no one coughing. There was a memory, or a name, or an image that arrived on its own, and suddenly you feel dirty. Inside, somehow. Tainted in a way that washing does not reach, though you may wash anyway, for a long time, because it is the only thing you know to do.

This is mental contamination, and I want to describe it carefully, because people who have it often do not recognise it as OCD, and often do not tell anyone, because it sounds impossible to explain.

What it is

Contamination OCD is usually pictured as fear of germs: physical contact, physical dirt, physical washing. Mental contamination is the same feeling without the physical route. The trigger is internal or moral: a memory of something that happened to you, a thought you find repugnant, a person who has hurt you, a place associated with something shameful. The feeling that follows is of being polluted, unclean, contaminated from within.

It is a recognised OCD presentation, and it is treated the same way as contamination by contact, because underneath, the mechanism is identical.

Why it makes the mechanism visible

In some ways mental contamination shows what all contamination OCD is really about. With door handles, it is possible to believe the feeling is about germs. With mental contamination, there were never any germs. The feeling of dirtiness was always generated by the fear, and the washing was always an attempt to remove a feeling rather than a substance. That is why it never worked, and why it never feels finished.

The compulsions

The compulsions in mental contamination can be physical: showering, changing clothes, cleaning the room where the thought arrived. They can also be mental: replacing the bad image with a good one, mentally cleansing, praying, reviewing the memory to establish that it was not your fault, avoiding anything associated with the trigger. And they can be about people: avoiding someone who feels contaminating, or the places and objects connected with them.

Each brings relief. Each teaches the brain that the feeling was a real threat that needed removing, which makes it arrive faster next time. This is the OCD engine, and it is not fussy about whether the contaminant is a germ or a memory.

Where it often comes from

Mental contamination is more often than other OCD themes linked to something that actually happened: a betrayal, an assault, a humiliation, a relationship in which you were treated as less than a person. The feeling of being tainted can be a residue of that, attached to the memory and to anything that recalls it. Where that is the picture, the work addresses both the OCD mechanism and the memory underneath, and EMDR can be part of it, once there is enough stability to work with.

What treatment does

The treatment is CBT with exposure and response prevention. We map the triggers, the feeling, and everything you do to remove it. Then, gradually and by agreement, we approach the triggers and let the feeling be there without the cleansing, physical or mental. The feeling rises, and then, given nothing to confirm it, it falls on its own, and the brain records a different lesson.

This is done at your pace. The first sessions are about understanding, not touching or approaching anything. You build the ladder, and you decide when to move up it.

Saying it out loud

People with mental contamination often carry it silently for years, because "I feel dirty when I think about my ex" or "I feel contaminated by a memory" sounds like something nobody will understand. I understand it. It is a recognised presentation, I have worked with it, and nothing you describe will be met with confusion or judgement.

If the dirt is inside and the shower never reaches it, there is more on how I treat contamination OCD, including the mental kind.
