Depression: the feeling follows the doing, not the other way round
Depression tells you to wait until you feel like it. The evidence says the feeling comes after the activity. Behavioural activation is why the early weeks of treatment are small and practical.

Everything takes more than it should: getting up, answering a message, making food, the shower you keep meaning to have. The things you used to enjoy are still there, and you feel nothing much when you do them, so gradually you stop. And when someone suggests doing more, the answer is obvious: I will, when I feel like it. When I have the energy. When this lifts.
That reasonable-sounding plan is the one depression wants you to follow, and the treatment starts from the opposite direction.
What depression tells you
Depression is persuasive, and one of the things it says most convincingly is: wait. You do not feel like seeing anyone, so do not. You will not enjoy the walk, so stay in. Rest, and when you feel better, you can start again. It sounds like self-care, and it is a trap, because the feeling does not come first.
What the evidence says
In depression, the feeling follows the doing. Mood lifts after activity, not before it. Waiting to feel like it means waiting for something that will not arrive on its own, because the activity is what produces it. That is the basis of behavioural activation, which does most of the work in the early weeks of CBT for depression, and it is why those weeks look the way they do.
How the spiral works
Depression shrinks the world. You stop doing things, because they take too much and give too little. With fewer things happening, there are fewer sources of anything good: no contact, no accomplishment, no movement, no pleasure, however faint. Mood drops further. The commentary gets louder: lazy, useless, everyone would be better off. With less energy and a harsher voice, doing anything is harder still, so more gets dropped. The messages pile up unanswered. The invitations stop. The room that needs tidying reproaches you every time you walk past it, and each of these becomes evidence for the prosecution.
Depression keeps itself going through that loop, in which less activity produces lower mood, which produces less activity.
Starting from where you are
Behavioural activation reverses the loop, deliberately and in small, realistic steps: the step you can actually take this week, on a bad day, from where you actually are, rather than the grand plan you would manage if you were well. That might be one message answered. A shower on two days. Ten minutes outside. It looks trivial, and it is the first movement in the opposite direction.
The steps are chosen for two things: a little accomplishment and a little pleasure, or at least the possibility of it. We schedule them rather than leaving them to motivation, because motivation is what depression has taken. And we record what actually happened, including how you felt afterwards, because depression is a poor judge of its own progress and the record usually notices before you do.
The commentary
Alongside the activity there is the thinking: the hostile commentator that runs all day and goes unchallenged because you are too tired to argue. Positive thinking does not touch it, and nobody with depression finds it convincing. What works is learning to notice the harshest conclusions as they arrive and to check them against what is actually true, which, once the activity has started to produce some evidence, becomes possible.
When it needs more
